At Access Plus we believe feedback allows us to resolve any issues you may have and gives us an opportunity to improve the services that you receive. Feedback includes a compliment, comment or a complaint. When you give us feedback we will treat you with respect. Your feedback will be kept safe, private, and protected.
If you would like to make a complaint, compliment or suggestion about our service, it is always best to first talk directly to the people who are providing your service, however, we understand that this may not always be possible. In that case, please complete the form down below and we will address the issue you raise. We will also contact you to find out more or to tell you the progress of your complaint, if you wish for us to do this you will need to provide your contact details.

Feedback & Complaints Policy

How can I submit feedback or complaints?
You can give us a call on (08) 9441 2677. This is a direct line to the office and you can speak to an independent member of staff about your concerns.

If you want someone else to help you with a complaint, but not someone from Access Plus, you can contact the Health and Disability Services Complaints Office (HaDSCO). HaDSCO is an independent authority which collects information about complaints in disability services and if necessary, provides an impartial resolution service. You can report any complaint directly to HaDSCO using this button.
